Two things here. First, on your takeoff, you lean back when you plant your right foot - that's good. However, you never convert your momentum upwards into the jump. Your upper body stays leaning back the entire time. This also causes you to fall after the kick.
Second, your left leg comes down when you kick with the right. Try to lean forward a bit when you kick with the right leg (and touch the right foot with both hands). You'll need to focus really hard on keeping the left leg up - that's the hard part of the kick.
The last thing is that your takeoff rotates too much in my opinion. I know that many people teach it that way. However, from a physics/mechanical standpoint, that's not efficient. The details are in the book but in short, try swinging the left leg across your midline during the takeoff. How do I convert my momentum upwards? |

You convert your momentum upwards just like you do in the jump front kick. Swing your arms and left leg up. Try to jump up, not forward. The takeoff for the two techniques are basically the same.
